I had a same case when I reconfigured the device powered by battery to be powered from the power supply. I won't check now exclusion / inclusion because I won't "get to" the sensor. It's on the roof as a UV sensor for comparison with the one on ESP8266.

No this device was configured via batt more than 1y ago on hc2

now this device transferred to HC3 with same issues

Device will add interface battery if battery CC was in Node Information Frame (NIF) during inclusion.
Also it will be added as sleeping slave or routing slave according to Listening flag in NIF.

As far as I know multisensor gen5 can be added both ways, controller configure the device according to data received during inclusion interview - if there was not "battery" CC in NIF then there won't be an interface for it and no battery indicator.

Try to factory reset the module when batteries are inside - maybe it will trigger the module to "refresh" current configuration.
Or there is a special procedure to change it to battery operated mode - have you checked devices manual?

Unfortunately no mention about including the device as powered by USB or battery...
    There is different manual:

Please login or register to see this link.

     - there is an information about this there.
No special procedure, just changing power source before inclusion.

This looks like an issue with module firmware, maybe try connecting USB power -> add it this way.
Then exclude it and try again but with batteries this time.

Not sure why this does not work correctly with your module, we use the same mechanism in our flood sensor and old smoke sensor - works ok.

    1) resetting

    2) added

That is what I always say about this module, not sure what is going on here but I can confirm what @10der posted: no battery icon. This is ZW5 fw 1.3 and it was never included on USB power, always ran on batteries on my HC2 then mograted to HC3. Did not try reset but as @10der already posted "does not help"...

@m.roszak If you want me to investigate, I can exclude and include and capture the interview process.

Mmmmm... Neither the template nor the entry on the Alliance website list many command classes, that is odd. It should have wake up command class, but

Please login or register to see this link.

says "Controlled Command Classes         Command Class Basic" - that's all. Matches the template. Weird. No "Controlling" part.
