I have a Fibaro Double relay switch (within a set up of many Fibaro switches controlled by Vera 3) that has this problem

 

O2 is working no problem while O1 is always short circuited and wont turn off.. meaning if i connect any Light to O1 it will always be turned on.

Also on Vera if i exclude and include the Switch or delete the device and include again...O1 will not give feedback that its on.
And turning it on and off through Vera doesnt make any difference on the relay hardware wise (as in it doesnt make clicking sound nor changes the state)

 

could this be a hardware problem that the Relay no1 is stuck? and if so..how to fix this?

I've got the same problem. I am using Fibaro HC2. Light stays on and and doesn't react on manual switch or command from HC2. In HC2 everything looks ok and you tthink you can operate it. It shows on and off, but light stays on all the time, HC2 doesn't show any communication error. Problem is only with O1, O2 works fine. I have a problem with 2 of those switches. It doesn't happen everytime, but couple times a day.

To me it looks like hardware problem, but I would like to confirm it.

Try knocking your module to demagnetise the switch. Its call latching if im not wrong.
