CP plus ip camera jpg path and mjpg stream path for HC2

Does anyone have a suggestion how to properly set up a jpg path and mjpg stream path for a  CP Plus  CP-UNC-DA20L3S-V2 camera in the HC2?

I am unable to connect to the HC2.
