Smart Implant Input 2 (IN2) Not Working

I am trying configure my Smart Implant to use both binary inputs.  I've wired it per the documentation (Section 4.7).  I have configured Parameter 20 and 21 as 0 (normally closed).  IN1 (yellow) works fine but IN2 (green) does not.  As a test I reversed the inputs using Parameter 24 and now IN2 (green) works but IN1 (yellow) does not.  I believe my wiring is correct and that I am missing a configuration.  How can I configure the Smart Implant to use both binary inputs.  I've searched and searched but have any suggestions would be much appreciated. 

 

My zwave radio is a Nortek GoControl HUSBZB-1 (USB) and Home Assistant which is successfully controlling 7 other zwave devices (switches/lights no inputs).
